Applying for a child care subsidy, a step-by-step guide for parents

Step 1: Choose a childcare provider

Step 2: Fill out the Child Care Arrangement Form
You and the provider you have chosen must complete the Child Care Arrangement Form together.

Step 3: Fill out the Child Care Subsidy Application form and gather any relevant supporting documents.

There are a number of supporting documents you will be required to submit depending on the specifics of your family. If you chose to submit by mail, always send copies and not the original.

  • ID: Identification for all family members must be provided. For this, you should submit a copy of government-issued ID.
  • PROOF OF STATUS: If relevant, you’ll be required to submit proof of citizenship status in Canada. For this, you can submit a copy of any formal document issued by Citizenship and Immigration Canada that confirms your status in Canada.
  • PROOF OF REASON AND INCOME: You must submit documents that support your responses in Section 2 (Reasons for Needing Child Care) and Section 3 (Income) of the Child Care Subsidy Application. A Supporting Documents PDF can help you figure out which documents can be used for this purpose.
  • SPECIAL NEEDS FORM: If you are a parent of a child with special needs, you will also need to submit a Special Needs Form.

Step 4: Submit!
Submit all documents – the Child Care Arrangement Form, Child Care Subsidy Application, and all supporting documents –  to the Child Care Subsidy Service Centre.
